Registration & Forms

Welcome to Mighty Mites Registration! You can register online or via paper. All liability and safety forms can be sent via the mail, or brought with you to the first day of Mighty Mites on December 4th.

Required forms for each child:

	•	Mighty Mite Registration Form:
		• NEW: Online Registration  Form!   • Downloadable PDF Mighty Mite Registration Form
	•	Alyeska Ski Club Acknowledgement of Risk and Release form
	•	Alyeska Resort Liability Release and Indemnification Agreement
	•	Read and sign “Know the Code” (Alyeska Resort skier responsibility code)
	•	Read the sign “Safety Violation Enforcement Guidelines”
	•	Each family must complete the: “Alyeska Resort Mountain Education Test”
	•	Fee is $175 per child
	•	Please print and sign all forms and either send to: Alyeska Mighty Mites PO Box 112322 Anchorage, AK 99511 Or bring them with you to the Race Training Center the first day of training on December 4, 2011.
